When I view your school site with your account I can see that you have 3 affective evaluations completed, 1 pending request, and 1 expired request. Is there a specific affective evaluation you cannot see? If yes, who are the preceptors for your missing evaluations?

When I look at your account I see that Jill Hayden has not completed your eval yet which is why you do not have a completed affective evaluation by her yet. You can see this on your "Affective Evaluations Requests" screen. I included the screenshot below. It also looks like Heather Adams never finished your eval request and incomplete requests are deleted after 30 days.
